【Abstract】Objectives:To evaluate the etiological diagnostic value of magnetic resonance imaging (MRI) in persistent and refractory hematospermia by analyzing their pelvic MRI manifestations and characteristic changes. Methods:The clinical data, bilateral seminal vesicles(SVs)and ejaculatory duct area morphological features of pelvic MRI images of 102 cases of persistent and refractory hematospermia patients were retrospectively analyzed. Results:Out of the 102 patients underwent MRI examination, 60 cases showed the typical change of the signal intensity in SV,in which 29 cases with fresh hemorrhage showed high signal intensity on T1-weighted images ( T1WI ),and low signal intensity on T2-weighted images (T2WI ), while the 31 cases with old hemorrhage showed middle to high signal intensity on both T1WI and T2WI; 32 cases showed significant cystic dilatation of unilateral or bilateral SVs (the width is bigger than 1.7cm), with or without the internal signal intensity change, 27 cases showed the prostatic utricular cysts, 5 cases show the ejaculatory duct cysts, 4 cases showed the Mullerian cysts, 1 case showed the SV cyst. 14 cases did not show any obvious abnormality in SV area by comparing with the normal pelvic MRI images of 60 cases of health adult men. Conclusion:Three-dimensional MRI tomography has a good resolution and can clearly display the pelvic soft tissue and refined structure, such as SV, prostate, vas deferens and ejaculatory duct, and has important etiological diagnostic value for the persistent and refractory hematospermia patients and also provide a reliable basis for subsequent treatment.
